Basically this song is in the position of any person who is in an abusive relationship. Since most abusive relationships may start at a young age, even the opposition may not understand how they can be hurting the person they care for. It's called "It's Up To You" because essentially it is up to that person to change for the better and learn from their mistakes and move on with it. It can also point back at the person who is being abused to think about the fact that they don't have to let their boyfriend or girlfriend treat them that way.
